ABSTRACT

Objective@#Despite the widespread use and measurement of beta‑human chorionic gonadotropin (β‑HCG) among hydatidiform mole (HM) patients, models derived from this biomarker to predict the remission or postmolar gestational trophoblastic neoplasia (GTN) rarely perform well. The study aimed to generate cutoff points for postevacuation β‑HCG levels and evaluate their performance among women with complete molar pregnancies@*Methods@#A retrospective cohort study composed of women with complete HM underwent bivariate procedures comparing characteristics between the comparison groups. Cut points using Liu’s and Youden’s indices were estimated, and their performance was evaluated using receiver operating characteristic curve analysis. Cox regression to compare time‑to‑progression across these proposed β‑HCG cutoffs was also performed.@*Results@#The incidence of postmolar GTN among the 155 women in the study was 15.5% (95% confidence interval: 10.2%–22.2%). Postevacuation HCG levels had a better prediction of disease status than preevacuation and HCG ratio models (χ2 : 163.07, P < 0.01). A cutoff at 508 mIU/mL the 3rd‑week postevacuation (area under the curve [AUC]: 0.89, sensitivity: 87.5%, specificity: 90.1%) was comparable with the 185 mIU/mL cutoff at the 5th‑week postevacuation (AUC: 0.89, sensitivity: 91.7%, specificity: 87%). The hazards ratio of postmolar GTN was 29.74 (8.53–103.71) and 39.89 (8.82–180.38) for the 3rd and 5th weeks HCG after evacuation adjusting for clinically relevant variables@*Conclusion@#The first 3rd‑ and 5th‑week postevacuation levels of β‑HCG demonstrated potential in predicting postmolar GTN. However, further refinement and adjustment for clinically relevant risk factors are still needed.

ABSTRACT

Introduction: there is a great diversity in the profile of cancers in the world. This study set out to analyze the profile of gynecological cancer in Federal University Teaching Hospital, Owerri, [FUTHO] (former Federal Medical Centre, Owerri, Imo state, Nigeria). Methods: this was a retrospective cross sectional descriptive study of the records of women admitted in the gynecological ward in FUTHO from January 2020 to November 2022. It was analyzed using SPSS version 23.0 and reported in simple percentages for categorical variables and measures of central tendency for quantitative variables. Results: a total of 1,378 gynecological patients were admitted into the Gynaecological ward of the hospital, out of which 242 (17.6%) were cancer cases. The most common cancer over the three years in review, was ovarian, 81(33.5%), followed by cervical, 66 (27.3 %), endometrial, 65 (26.8 %), choriocarcinoma, 22 (9.1%), vulvar, 6 (2.5%) and vagina, 2(0.8%). The most common gynecological cancers in this study is very different from previous reports from Nigeria and other African countries. The pattern looks like that seen in the developed countries where endometrial and ovarian cancers top the list. Conclusion: this report shows a possible change in lifestyle and improved access to cervical cancer prevention strategies. It is also assumed that all the facilities who have recorded cervical cancer as the most common cancer can actually have a similar result as ours if a more current review is done.

ABSTRACT

A doença trofoblástica gestacional (DTG) agrupa um conjunto de anomalias do desenvolvimento trofoblástico, que incluem formas clínicas benignas como a mola hidatiforme completa e parcial, o nódulo do sítio placentário atípico e o sítio trofoblástico exagerado, e malignas, caracterizando a neoplasia trofoblástica gestacional (NTG). De modo geral, seu diagnóstico precoce antecipa complicações clínicas que podem estar associadas a near miss obstétrico. Diante da suspeição clínica, é a ultrassonografia (US) precoce o exame de escolha pa ra o diagnóstico, associado à dosagem sérica de gonadotrofina coriônica humana, capaz de minimizar a ocorrência de complicações clínicas associadas à gravidez molar. Nos casos de NTG, é a US também de grande valia para estadiamento, avaliação de prognóstico e acompanhamento da mulher tratada para DTG. Este estudo faz uma revisão sobre o papel da US na DTG, sendo importante para familiarizar os tocoginecologistas com essa doença e salientar o papel da US consoante as melhores práticas clínicas.(AU)


Gestational trophoblastic disease (GTD) includes a set of trophoblastic developmental anomalies, which include benign forms such as complete and partial hydatidiform mole, atypical placental site nodule and exaggerated trophoblastic site, and malignant forms, characterizing gestational trophoblastic neoplasia (GTN). In general, its early diagnosis anticipates clinical complications that could be associated with obstetric near miss. In view of clinical suspicion, early ultrasonography (US) and serum levels of human chorionic gonadotropin are the best diagnostic screening techniques, able to minimizing the occurrence of medical complications associated with molar pregnancy. In cases of GTN, US is also of great value for staging, assessment of prognosis and follow-up of women treated for GTN. This study reviews the role of US in GTD, being important to familiarize tocogynecologists with this disease and highlight the role of US according to best clinical practices to minimize the morbidity of these patients and maximize the remission rates of this disease.(AU)

ABSTRACT

@#Extrauterine choriocarcinoma is a rare entity. The criteria used for its diagnosis are as follows: (1) Absence of disease in the uterine cavity, (2) pathologic confirmation of diagnosis, (3) exclusion of molar pregnancy, and (4) absence of a coexistent intrauterine pregnancy. Delay in the diagnosis can be attributed to its nongynecologic manifestations such as bleeding from any organ system, unexplained systemic symptoms, and metastatic foci from an unknown primary malignancy. This is an unusual case of 27-year-old G3P3 (3-0-0-3) who underwent emergency left parietal craniotomy excision due to increased intracranial pressure symptoms secondary to left parietal tumor. Histopathology revealed metastatic adenocarcinoma. About a month later, she underwent exploratory laparotomy for acute abdominal symptoms secondary to a jejunal mass. Jejuno-jejunal resection anastomosis was done and histopathology revealed choriocarcinoma.

ABSTRACT

@#A 28-year old, G5P4 (4014), noted neck lymph nodes associated with cough. A chest X-ray was done showing a left nodular opacity. Antibiotics were prescribed with a resolution of symptoms. Five months after, a routine chest X-ray revealed interval progression in size of the lung nodule. A chest computed tomography (CT) scan and positron-emission tomography scan were done subsequently showing the precise location and size of the nodule and with no other focus of tumor seen. Transvaginal ultrasound was normal. With an initial diagnosis of lung carcinoma, a percutaneous needle aspiration biopsy under CT scan guidance was done. Immunohistochemical staining panel showed that beta-human chorionic gonadotropin (hCG) was positive. Subsequently, a serum beta-hCG done showed low levels from 33.48 to 59.7 mIU/ml. The final diagnosis given was a poorly differentiated malignancy highly suggestive of malignant trophoblastic tumor. A video-assisted left upper lobectomy was performed with histopathology and immunohistochemistry consistent with epithelioid trophoblastic tumor with co-existing choriocarcinoma elements. Postoperative beta-hCG level dropped to normal and remained so for 2½ years.

ABSTRACT

@#Gestational trophoblastic neoplasias (GTN) are extremely aggressive tumors derived from placental trophoblasts. These tumors are always the sequalae of a pregnancy. Choriocarcinoma, which is the most common of these, is typically characterized by early extra-pelvic hematogenous spread. Since the progression of illness is rapid, timely diagnosis and treatment will favor improved chances for cure, whereas late commencement of therapy will make resolution difficult. The diagnosis of GTN is straightforward with an elevated beta-human chorionic gonadotropin (β-hCG) and distinct sonographic features of the tumor inside the uterus. However, very rarely, this disease may occur in the absence of uterine tumors. Practicing physicians must be mindful that GTN may initially manifest with pulmonary symptoms and/or radiographic evidence of metastatic lung lesions. In this series, the features pertaining to the clinical course of three patients are described, all of whom presented with pulmonary masses, elevated β-hCG, and normal transvaginal sonograms.

Introducción: El coriocarcinoma es una neoplasia gestacional trofoblástica maligna que puede ocurrir después de una mola hidatiforme, embarazo ectópico, aborto e incluso de un embarazo normal. Se trata de una patología poco frecuente que en ocasiones puede pasar desapercibida. Entre las complicaciones clínicas se destaca la metástasis, pero es excepcional la ruptura uterina, siendo esta la que se presenta en el presente caso denotando la importancia del seguimiento. Presentación de caso: Paciente femenina de 49 años que presenta dolor en hipogastrio y vómito, tiene antecedente de mola hidatiforme no controlada. Examen complementario muestra beta-gonadotropina coriónica humana (B-HCG) cualitativa positiva y culdoscentesis positiva. Se realiza tratamiento quirúrgico por laparotomía exploratoria evidenciándose hemoperitoneo más tumor que perfora útero. Se realiza histerectomía abdominal total. Se confirma diagnóstico presuntivo a través de estudio histopatológico que determina coriocarcinoma. Paciente no continua con tratamiento conociéndose posteriormente su deceso. Conclusiones: Se presenta este caso por ser una patología infrecuente con una complicación poco habitual como es la ruptura uterina por coriocarcinoma, al ser considerada una rareza su importancia radica en el diagnóstico oportuno y adecuado seguimiento del caso.

Paciente de 48 años que cursa con dolor abdominal asociado a perdida ponderal. En la endoscopía se evidencia una lesión una lesión infiltrativa ulcerada, friable y circunferencial que compromete el canal pilórico y bulbo duodenal Al estudio histopatológico se revela un adenocarcinoma tipo intestinal con área de diferenciación en coriocarcinoma. Se realizó una gastrectomía parcial distal y posteriormente quimioterapia dirigida a la estirpe del coriocarcinoma, con buena respuesta clínica al primer curso. (AU)


A 48 year-old patient with abdominal pain associated and weight loss is shown. Endoscopy shows an ulcerated, friable, circumferential infiltratory lesion that compromises the pyloric canal and duodenal bulb The histopathological study reveals intestinal type adenocarcinoma with area of differentiation in coriocarcinoma. A partial distal gastrectomy and subsequent chemotherapy were performed aimed at the lineage of the coriocarcinoma, with good clinical response to the first course. (AU)

@#Gestational trophoblastic neoplasia is a group of tumors which includes invasive mole, choriocarcinoma, placental site trophoblastic tumor and epithelioid trophoblastic tumor, all of which develop after a recognized pregnancy. Choriocarcinoma is a highly invasive and metastatic neoplasm which arises in women of reproductive age. Local spread is reported at 15% while distant metastasis at 4%. Of the 4% of cases having distant metastasis, 60% goes to the lungs, 30% to the vagina, and 10% to other sites. Less than 5% of patients with metastatic gestational trophoblastic neoplasia have involvement of the gastrointestinal tract. This is the case of a 47-year-old multigravid patient who came in with an enlarging abdomen 8 years after she had a hydatidiform mole. Work-ups were done which revealed metastases to the colon, liver and lungs. The plan of management was to give multiple agent chemotherapy.

@#Term pregnancy with choriocarcinoma is a rare entity and cases where both the mother and fetus survive are even more rare. This paper discusses the case of a term pregnancy with a concomitant choriocarcinoma in a 21 yearold, gravida 2 para 0, who presented with hematochezia on the third trimester. She delivered via cesarean section for non-reassuring fetal status with good neonatal outcome. She underwent exploratory laparotomy postdelivery due to profuse gastrointestinal bleeding associated with deteriorating hemodynamic status. Histopathologic report revealed Ileal Choriocarcinoma. Metastatic work up showed liver and lung metastasis. The patient achieved remission after 8 cycles of EMACO chemotherapy. There was no evidence of recurrence in the subsequent 10 months of regular follow up.

ABSTRACT

Extragonadal non-gestational choriocarcinoma (ENC) is an uncommon malignant tumor occasionally found in the gastrointestinal tract. ENC is characterized by a biphasic tumor growth with distinct areas of adenocarcinoma and choriocarcinoma differentiation. Primary choriocarcinoma of the colon is extremely rare, with only 21 cases reported in the literature. Most of the perforation of colorectal cancers occurs in the abdominal cavity, while abdominal wall abscess is rare; the psoas abscess associated with colon carcinoma is even less observed. Herein, we report the case of a 61-year-old female with poorly differentiated adenocarcinoma of the ascending colon and sigmoid, with choriocarcinomatous differentiation, masquerading a psoas abscess formation. Unfortunately, despite the aggressive therapy, the patient's disease rapidly progressed, and she died within 2 months after the diagnosis. The typical morphological pattern, immunohistochemistry, and its correlation with serum β-human chorionic gonadotropin enabled a correct diagnosis.

ABSTRACT

Doença trofoblástica gestacional (DTG) é uma anomalia que engloba formas clínicas benignas (mola hidatiforme completa e parcial) e malignas (mola invasora, coriocarcinoma, tumor trofoblástico do sítio placentário e tumor trofoblástico epitelioide). O objetivo deste estudo é realizar levantamento epidemiológico retrospectivo de prontuários de 40 pacientes internadas entre abril de 2014 e fevereiro de 2016 com hipótese diagnóstica de DTG atendidas no Hospital Regional Norte/ Centro de Apoio à Saúde Reprodutiva da Mulher em Sobral, no Ceará, traçando o perfil de cada paciente (idade, paridade), além de fazer correlação dos parâmetros clínicos, laboratoriais e anatomopatológico. Entre as pacientes que obtiveram o diagnóstico de DTG, observou-se que em torno de 93,33% possuíam exame ultrassonográfico evidenciando possível mola hidatiforme; o anatomopatológico confirmou doença trofoblástica em aproximadamente 52,5% da população estudada. Este estudo é inédito, por ser o primeiro a realizar um levantamento de dados em pacientes com DTG na cidade de Sobral.(AU)


Gestational trophoblastic disease (GTD) is an anomaly that encompasses benign clinical forms (complete and partial hydatidiform mole) and malignant (invasive mole, choriocarcinoma, placental site trophoblastic tumor and epithelioid trophoblastic tumor). The objective of this study was to carry out a retrospective epidemiological survey of medical records of 40 hospitalized patients between April 2014 and February 2016 with diagnostic hypothesis of GTD attended at the Regional Hospital Norte/Center for Support to Women's Reproductive Health in Sobral, Ceará, drawing the profile of each patient (age, parity), in addition to correlating the clinical, laboratory and anatomopathological parameters. Among the patients who had the diagnosis of GTD, it was observed that about 93.33% had ultrasonographic examination evidencing a possible hydatidiform mole; the anatomopathological confirmed trophoblastic disease in about 52.5% of the study population. This study is unprecedented because it is the first to perform a data collection in patients with GTD in the city of Sobral.(AU)

ABSTRACT

El coriocarcinoma gástrico primario (CGP) es un tumor infrecuente. Debido a su rareza, su patogenia aún no es clara. El diagnóstico se basa en la inmunohistopatología, la cual es positiva para gonadotropina coriónica humana (hCG). Es una neoplasia altamente invasiva y de rápida diseminación hematógena, lo que conlleva a un pobre pronóstico. Se presenta el caso de una paciente mujer de 57 años con hemorragia digestiva alta asociada a baja de peso. La gastroscopia reporta, a nivel de cardias y región paracardial, lesión polipoidea ulcerada de 5x4 cm, con bordes circunscritos y sin presencia de infiltración de pared adyacente, con estigmas de sangrado reciente y anatomía patológica que informa neoplasia maligna pleomórfica ulcerada con inmunohistoquímica positiva para panqueratina. Ante la sospecha de carcinoma poco diferenciado y sin evidencia de metástasis, la paciente fue sometida a gastrectomía total y la patología fue compatible con coriocarcinoma. Por ello, posterior a la cirugía, se realiza estudio de hCG sérico con resultado de 714 mIU/ml, lo que confirmó aún más el diagnóstico. Se decide tratamiento adyuvante con quimioterapia y se realiza seguimiento tomográfico y serológico de hCG sin presencia de enfermedad activa.


Primary gastric choriocarcinoma (PGC) is an uncommon tumor. Due to its rarity, its pathogenesis is still unclear. The diagnosis is based on immunohistopathology, which is positive for human chorionic gonadotropin (hCG). It is a highly invasive and rapidly-disseminated hematogenous neoplasm, which leads to a poor prognosis. We present the case of a 57-year-old woman with upper gastrointestinal bleeding associated with weight loss. The gastroscopy showed, on the cardia and paracardial region, a 5x4-cm ulcerated polypoid lesion with circumscribed edges and without adjacent wall infiltration, with stigmas of recent bleeding and a pathological anatomy that demonstrates ulcerated pleomorphic malignancy with positive immunohistochemistry for pankeratin. Given the suspicion of poorly differentiated carcinoma and without evidence of metastasis, the patient underwent a total gastrectomy and the pathology was compatible with choriocarcinoma. Therefore, after the surgery, a serum hCG test was performed with a result of 714 mIU/ml, which further confirmed the diagnosis. Treatment with adjuvant chemotherapy was decided, and a tomographic and serological hCG follow-up was conducted, without the presence of active disease.

Resumen Se publica el caso de una paciente de 27 años, quien consultó al Servicio de Urgencias del Hospital Hernando Moncaleano Perdomo, por cuadro clínico de tos seca persistente que posteriormente se acompañó de disnea, dolor pleurítico y hemoptisis, de aproximadamente cuatro meses de evolución. Tenía antecedentes quirúrgicos de lipoescultura y abdominoplastia cuatro meses previos al ingreso e histerectomía hace un año (por hemorragia uterina). Mediante correlación clínico-patológica se llegó al diagnóstico de embolia pulmonar de origen tumoral, entidad aún de diagnóstico difícil dada la presentación clínica inespecífica. Para la diferenciación diagnóstica los estudios radiológicos no son precisos, a menos que se lleven a cabo procedimientos invasivos para llegar a un diagnóstico certero, como ocurrió con la paciente. Se presenta el caso y se hace una revisión sobre embolia pulmonar y sus otras causas como coriocarcinoma; adicionalmente se abarcan aspectos relevantes sobre incidencia, cuadro clínico, tratamiento y pronóstico, de una patología cuyo diagnóstico sigue siendo en su mayoría tardío.


Abstract The case is presented of a 27 year-old patient who was seen in the Emergency Department of the Hospital Hernando Moncaleano Perdomo, due to a clinical picture of a persistent dry cough that was later accompanied by dyspnoea, pleural pain, and haemoptysis, of approximately four months onset. She had a surgical history of liposculpture and abdominoplasty four months prior to the admission, and a hysterectomy a year ago (uterine haemorrhage). Using clinical-pathological correlation, the diagnosis was made of a pulmonary embolism of tumour origin, a condition still difficult to diagnose given the unspecific clinical picture. Radiological studies are not accurate for the differential diagnosis, unless invasive procedures are used to reach an accurate diagnosis, as in the case of this patient. As well as the case, a review is presented on pulmonary embolism and its other causes such as choriocarcinoma. Furthermore, relevant aspects are included on its incidence, clinical symptoms, treatment and prognosis, in a disease for which the majority of diagnoses continue to be delayed.

ABSTRACT

Choriocarcinoma is an aggressively growing and widely metastasizing tumor that originates from trophoblastic cells. A primary gastric choriocarcinoma (PGC), however, is very rare. A 76-year-old female patient visited the emergency department of Wonkwang University Hospital with abdominal discomfort and melena. Esophagogastroduodenoscopy revealed a huge ulceroinfiltrative mass lesion with blood clots on the boundary between the greater curvature side and the posterior wall side of the stomach lower body. CT showed a 3-cm exophytic mass lesion with heterogeneous enhancement, an ulcer lesion at the posterior wall side of the stomach lower body, and multiple enlarged lymph nodes at the splenic artery and left gastric artery nodal stations. She underwent a radical subtotal gastrectomy with a D2 lymph node dissection. The final diagnosis was PGC coexisting with adenocarcinoma based on the pathology results. This paper reports a rare case of primary gastric choriocarcinoma coexisting with adenocarcinoma and discusses the characteristics of this neoplasm with reference to the literature.

ABSTRACT

Choriocarcinoma is an aggressively growing and widely metastasizing tumor that originates from trophoblastic cells. A primary gastric choriocarcinoma (PGC), however, is very rare. A 76-year-old female patient visited the emergency department of Wonkwang University Hospital with abdominal discomfort and melena. Esophagogastroduodenoscopy revealed a huge ulceroinfiltrative mass lesion with blood clots on the boundary between the greater curvature side and the posterior wall side of the stomach lower body. CT showed a 3-cm exophytic mass lesion with heterogeneous enhancement, an ulcer lesion at the posterior wall side of the stomach lower body, and multiple enlarged lymph nodes at the splenic artery and left gastric artery nodal stations. She underwent a radical subtotal gastrectomy with a D2 lymph node dissection. The final diagnosis was PGC coexisting with adenocarcinoma based on the pathology results. This paper reports a rare case of primary gastric choriocarcinoma coexisting with adenocarcinoma and discusses the characteristics of this neoplasm with reference to the literature.

ABSTRACT

@#Hydatidiform mole (H mole) is a rare complication of pregnancy, characterized by an atypical trophoblastic proliferation and swelling of the chorionic villi, with or without a live fetus. A clinical diagnosis of H mole is confirmed by significantly high serum beta-hCG levels and/or the presence of characteristic sonographic and histopathologic findings. Differentiation between the two types—complete or partial—is important, since complete moles pose a higher risk for malignancy. An H mole can be further complicated by the presence of a coexisting live fetus. Complete molar pregnancy with a coexisting live fetus has an incidence of only 1 in 22,000 to 100,000 pregnancies. Termination of pregnancy is usually considered, especially when a high risk of choriocarcinoma and maternal prenatal complications are present. We present the case of a 27-year-old female diagnosed as having a complete H mole with a coexisting live fetus. We terminated the pregnancy due to profuse vaginal bleeding that could cause maternal and fetal compromise.

ABSTRACT

El coriocarcinoma gástrico primario (CGP) es un tumor extremadamente raro, altamente invasivo y de rápida diseminación hematógena. Presentamos el caso de una paciente de 57 años que inicia con cuadro de hematemesis y, progresivamente, se le suman episodios de melena, baja de peso y epigastralgia. Es derivada al Instituto Nacional de Enfermedades Neoplásicas en donde se le realizan gastroscopía y biopsia. Así, el análisis histológico reportó patrón sugestivo para CGP; el cual se confirmó al realizarle a la paciente los estudios por imágenes necesarios y llevar a cabo el análisis inmunohistoquímico para gonadotrofina coriónica humana y alfa feto proteína. Posteriormente, a la paciente se le realiza una gastrectomía radical D2 con preservación esplénica y de cola de páncreas. Lamentablemente, su evolución no fue favorable y fallece por la progresión de la enfermedad.


Primary gastric choriocarcinoma (PGC) is an extremely rare and highly invasive tumor with rapid hematogenous spread. We present the case of a 57-year-old female patient who started with hematemesis and progressive episodes of melena, weight loss and epigastralgia. It is derived from the National Institute of Neoplastic Diseases where gastroscopy and biopsy are performed. Histological analysis reported pattern suggestive of PGC; that was confirmed by immunohistochemical analysis for human chorionic gonadotrophin and fetal alpha protein. Subsequently, the patient underwent a radical D2 gastrectomy with splenic preservation and tail of the pancreas preservation. Unfortunately, her evolution was not favorable and died due to the progression of the disease.

ABSTRACT

Resumen La enfermedad trofoblástica gestacional engloba un conjunto de patologías con potencial maligno y neoplásicas propiamente, las cuales pueden ser adquiridas tras la gestación. Aunque estos tumores abarcan menos del 1% de los tumores ginecológicos, representan una amenaza para la vida de las mujeres en edad reproductiva. Es importante que los médicos comprendan su etiología, evolución natural y manejo, debido a su alto potencial de curación con la posibilidad de preservar la función reproductiva si se diagnostica a tiempo y se trata adecuadamente según sus criterios de riesgo y pronóstico.


Abstract The gestational trophoblastic disease englobes a set of neoplastic pathologies and pathologies with malignant potential, which can be acquired after gestation. Even though these tumors include less than 1% of gynecological tumors, they represent a threat to the life of women in reproductive age. It is important that physicians understand its etiology, natural evolution and control. If it is diagnosed on time and treated appropriately according to its risk and prognosis criteria, there's a high healing potential including the ability of preserving reproductive function.
